When you submit a message voluntarily via our Contacts & Bookings page or make an appointment through our practice management software booking system, your name, email address, and phone number are requested as part of our provision of speech pathology services.
If you choose to become a client with us, we collect further data including (but not exclusive to) your date of birth, gender, pronouns, assigned sex at birth, addresses, contact details, Medicare number (for identification and claiming purposes) and medical information - including medical history, medications, allergies, social history, family history, In some cases, with your consent, we may also request relevant treatment notes and reports from other healthcare providers to help inform your assessment and therapy. You have the right to gain access to the information held about you with The Affirm Group.
